Individualized Medicine in Managing Patients with T2DM– Describe patient-specific factors that contribute to the pathogenesis and influence treatment outcomes in patients with T2DM.Assess how individualized therapy should relate to current treatment guidelines and algorithms in enabling effective T2DM patient management. Manage patients with T2DM based on individualized assessments. Provide appropriate care and counsel for patients and families with T2DM. Utilize the knowledge and skills of all members of a multidisciplinary, patient-centered diabetes treatment team.
