Case Managers and Their Role in Improving Patient Outcomes in Heart Failure– The purpose of this continuing education activity is to educate CCM® board certified case managers on the current management of heart failure (HF) and emerging therapies that may impact treatment paradigms and patient outcomes. As more research in the field of heart failure gives rise to the emergence of new treatment options, it is critical that case managers be up to date on the currently available treatments, emerging treatments, and the safety and effectiveness data.
-This educational activity will provide case managers with pertinent, clinically relevant and up-to-date information on novel therapies and their impact on future treatment paradigms and clinical practice.
Discuss current evidence-based approaches for the management of diverse patient with HF.
Identify HF patients with worsening HF who may require optimization of therapy due to lack of efficacy, tolerability, or nonadherence.
Evaluate the mechanisms of action, efficacy, and safety of emerging treatments for HF.
